Antibiotic Appropriateness Assessment Tool


PQC piloted a large-scale assessment of antibiotic appropriateness in participating intensive care units (ICUs) across 11 PQC-affiliated organizations on October 5, 2016. PQC adapted a CDC tool to determine whether antibiotics received on October 5, 2016 were classified as inappropriate and the reason for identifying the use as inappropriate. This PQC-adapted tool was refined with suggestions from the pilot and another antibiotic appropriateness assessment was completed on March 1, 2017. Twelve PQC-affiliated organizations participated with 47 ICUs. A poster presentation of this assessment was presented at IDWeek 2017 in San Diego on October 5, 2017.


If you would like to reproduce this antibiotic appropriateness assessment in your hospitals, we would recommend the following process:

  1. Identify participating ICUs and a specific day the assessment will be completed. Fill in the tools below with this date.
  1. Use the PQC Individual Antibiotic Appropriateness Assessment Tool to assess the antibiotic appropriateness for each patient in participating ICUs on antibiotics. Direct the Antibiotic Stewardship Program team to complete the tool.
  1. Complete the PQC Aggregate Antibiotic Assessment Tool to submit information for all patients reviewed in each participating ICU.
  1. Analyze information obtained from the Aggregate Tools to benchmark antibiotic appropriateness across hospitals.
  1. Allow the information from this assessment to guide inpatient Antibiotic Stewardship Programs.
